Pharmacy Technician-Transitions of Care

GENERAL JOB DESCRIPTION

Using a care transition model, work with patients being discharged from hospitals to reduce the chance of re-admission.  Focus is placed on medication usage, coaching and empowering patients to take an active role in their post-discharge recovery.  Provides patient information to clinical professionals. Effectively links patients and providers to available community-based resources by coordinating with area agency on aging staff.

DUTIES

  • Implement the Care Transitions model in accordance with project targets and projected outcomes.
  • Actively participate in team meetings by providing input in brainstorms, sharing case studies, reviewing readmission trends, and utilizing data feedback to improve processes and outcomes.
  • Develop and maintain open lines of communication with care transition hospital partners.
  • Attend and maintain Coleman Care Transition certification.
  • Work on-site at hospitals, adhering to care transitions policies & procedures which have been established with our organization and partner healthcare delivery organizations.
  • Conduct home visits with participants of the program, per requirements of the care transitions model.
  • Conduct follow-up telephone calls to participants of the program, per requirements of the care transitions model.
  • Collect and accurately enter all data into the Care Transition database.
  • Transmit patient medical information to patient’s clinical team.
  • Coach patients and caregivers on how to access available community resources and interact with healthcare providers as part of their healthcare team.
  • Other duties as assigned by a Manager, Department Director or the Executive Director.

REQUIREMENTS

MUST BE A LICENSED PHARMACY TECHNICIAN IN THE STATE OF MICHIGAN.  Has excellent interpersonal and group process skills.  Is accurate, conscientious and flexible with attention to detail. Is proficient with computers.  Accepts direction, is well organized, and has the ability to meet deadlines.  Willingness to work in a team environment.  Perform job functions with ethics, honesty and integrity.  This position is subject to the rules/regulations of hospital and national accreditation standards which at a minimum include: annual tuberculosis tests, flu vaccinations, and vaccinations (or proof of immunity) for MMR, Varicella, Hepatitis B, and potentially others as required.

EDUCATION

Prefer a Bachelors Degree in a Liberal Arts or Human Services field.

EXPERIENCE

Two years of experience in a professional work environment.  Desirable experience includes: customer service, patient/peer coaching, medical patient interaction, community relations, outreach, and data entry.
